The Carilion Clinic - Virginia Tech Carilion Geriatric Medicine Fellowship is comprised of 13, two- to four-week blocks.

Longitudinal Component

The longitudinal component includes an ambulatory and long-term care experience. Fellows will have a weekly half-day session in a geriatric-dedicated Internal Medicine clinic.

In addition, fellows will have one half-day sessions in the Carilion Center for Healthy Aging- Memory Disorder Clinic. 

Lastly, fellows will be assigned a continuity panel of patients at a regional Assisted Living and Memory Care facility. 

Post-Acute Long-Term Care

The post-acute long-term care rotation encompasses two blocks and includes experiences in both institutional and non-institutional settings. These blocks will take place at various regional long-term care facilities and the Salem Veterans Affairs Medical Center.

PACE

The PACE rotation (through InnovAge) is a half-block experience working with the program for all-inclusive care of the elderly that includes primary and specialty care, medications, care coordination, transportation, meals and facility-based rehabilitation. This program provides care to older adults in their homes and within an adult day center environment. 

Home Care

Home health services (i.e., nursing, physical therapy, occupational therapy, speech therapy, aides and medical social work) are instrumental in promoting recovery and function within the home. The home care rotation is a one-block experience in the home setting that provides fellows deeper insights into how these services can be applied to their future patient care.

Inpatient Consults

The consult service will be primarily based at Carilion Roanoke Memorial Hospital with an opportunity for expansion to one of our rural community hospitals. The two blocks will include inpatient geriatric consults requested throughout the hospital and co-management of older adults admitted to ED Observation. The fellows will make the initial contact when a geriatric consult is requested. 

The fellows will be involved in the initial care and continuing responsibilities for all consult patients and will participate on the inpatient multidisciplinary team.

Palliative Care

In this one block rotation, fellows will round with the Carilion Roanoke Memorial Hospital Palliative Care team caring for patients on the Palliative Care Unit and seeing inpatient consults. Fellows will also have the opportunity to work in our outpatient Palliative Care Clinic.

Inpatient Rehabilitation

The Rehabilitation and Wound Care rotation is half-block in the inpatient and outpatient Rehabilitation Unit. The fellows will be able to observe and participate in the subacute phases of care.

Neurology and Neuropsychology

One half block will be spent with Carilion Clinic Neurologists. The fellows will work in the outpatient setting, working with movement and memory specialists. Additionally, fellows will spend one-half block working with Neuropsychologists, learning about neuropsychiatric testing and evaluation.

Joints and Bone Health

This one-block rotation is an opportunity for fellows to follow Orthopedic Surgeons in the outpatient setting. There, fellows will see pre- and post-evaluation of common orthopedic procedures in older adults. Additionally, they will have the opportunity to develop proficiency in outpatient orthopedic procedures. Lastly, fellows will spend time learning the management of osteoporotic bone disease.

Geriatric Psychiatry

The Geriatric Psychiatry rotation is a one-block experience. This will include experiences with trained Geriatric Psychiatrists in the outpatient and long-term care setting.

Audiology

Fellows will spend a half-block rotation working with our Audiology clinic, learning about the basics of evaluative hearing exams and hearing aids.

Wound Care

Fellows will spend various Tuesday half-days with our outpatient Wound Care team to learn the basic and advanced wound care strategies.

Podiatry

Fellows will spend one-half block working with Podiatrists in the outpatient setting to learn the basics of foot care in older adults.

Scholarly Pursuits

Fellows will have one-block of designated time to pursue research or quality improvement interests of their choice, guided by a faculty mentor.

Electives

The fellows will have the opportunity to select two electives in fields of interest during the fellowship. Outside rotations are permitted if the discipline is not available through Carilion Clinic.
