Reporting a Concern

Carilion Clinic Medical Education is continually looking for ways to improve our policy and procedures concerning resident and fellow education and the medical education learning climate. There are several different methods by which a trainee may report concerns: 

  • Your Attending Physician
  • Your Program Director or Program Manager
  • The Designated Institutional Official: Arthur Ollendorff, MD, aollendorff@carilionclinic.org, 828-333-0609
  • Any Member of the GME Staff: 540-581-0322
  • SafeWatch Submission 

There may be times when residents and fellows would prefer to provide suggestion for improvements or complaints in an anonymous fashion. This can be done through a confidential Qualtrics report. The information you provide through the link below is completely anonymous and is sent to a non-medical education server where your identity is scrubbed from the email. When the comments are forwarded directly to the Designated Institutional Official, there are no means for determining the identity of the original sender. Please use your best judgement when submitting any comments. Your feedback will be read and taken seriously.
