Internal Medicine Residency
Our 4+1 Schedule
Our categorical Internal Medicine program utilizes a 4+1 scheduling model (sample schedule). Unlike a traditional 13, four-week block system that incorporates weekly half-day continuity clinics into existing rotation, this curricular design separates continuity clinic from required/elective rotations. The “X” block is four weeks in duration, which can be divided into two 2-week rotations or one 4-week rotation. The “Y” block, or ambulatory block, is one week in duration. The Y block incorporates a resident’s longitudinal ambulatory continuity clinic and also affords the opportunity to rotate in a subspecialty clinic of the resident's choosing. The resident's subspecialty clinic may arise from a resident's future career interest or from a desire to improve knowledge in a particular subspecialty.
Our preliminary residents are scheduled on a 13, four-week block system that does not include continuity clinic. This affords prelim residents 10 to 11 weeks of additional elective opportunity to ensure a broad base of medical knowledge before entering subspecialty training.
Benefits of a X+Y schedule includes:
- Decreases conflicting inpatient and outpatient responsibilities, thereby reducing resident stress and work hours while on inpatient rotations
- Decompresses a resident’s schedule by removing the possibility of back-to-back intensive rotations (wards, ICU, nights)
- Allows for dedicated, continuous outpatient continuity time without competing interests
- Increases ownership and satisfaction in the ambulatory setting
- Allows the ability to provide a dedicated ambulatory curriculum
